Our supports

Practical support, organised around everyday life

Our registration groups cover personal and household activities, community participation, nursing care and specialist accommodation. The right arrangement depends on the participant’s goals, NDIS plan and an agreed service plan.

    Daily living

    Personal activities, shared living and household tasks arranged around day-to-day needs.

    Community and independence

How support starts

A clear path from referral to agreed supports

These steps make space to understand the participant, check practical details and confirm consent before services begin.

  1. Make a referral

    A participant, family member, representative or professional can share the support request with us.

  2. Meet and agree on the support plan

    We discuss the participant’s goals, support needs, preferences and the details needed for a service agreement.

  3. Begin supports after details and consent are confirmed

    Supports can begin once the required information, consent and service arrangements have been agreed.

Make a referral

Tell us what support is being considered

Participants, families and representatives can refer. We also welcome referrals from support coordinators, LACs, plan managers and discharge teams.

It helps to include the participant’s goals, relevant support needs, contact preferences, plan management and consent to share the information.
