Community and independence

Community nursing related to disability needs, based on assessment and the participant’s plan.

Home and accommodation
Specialist disability accommodation for eligible participants where it is included in their plan.

The codes and titles below match the client-supplied registration groups exactly. Whether a particular support is suitable still depends on the participant’s plan, needs and agreed service arrangements.
Daily living
Personal activities, shared living and household tasks arranged around day-to-day needs.

- Assistance with personal activities where more complex support is required, shaped by assessed needs and the participant’s plan.

- Assistance with personal tasks and daily routines at the level agreed with the participant.

- Support with daily tasks in a shared living setting, based on individual routines and agreed assistance.

- Assistance with household activities to help maintain a safe and workable home environment.
Community and independence
Support with transitions, travel, skill development and participation in community life.

- Support to take part in social, recreational and community activities selected by the participant.

Clinical support
Community nursing related to disability needs, based on assessment and the participant’s plan.
